For Virtual Intake Appointments:
You will receive the intake paperwork to your email address. Once the intake paperwork is completed, your virtual intake appointment will be confirmed. If you do not receive the intake paperwork, please text 203-212-8842. We look forward to providing you with your treatment needs.
For Parent Education Program classes:
We will email you the payment form. We accept debit and credit cards. We accept Mastercard, Visa and Discover. American Express is not a form of payment accepted. Once payment is received, your registration will be complete. There are limited placements in the classes, therefore to secure a placement in this class, we recommend submitting payment form as soon as possible. If you do not receive the payment form within 24 hours, please text 203-212-8842. PLEASE DO NOT REGISTER FOR A CLASS MORE THAN ONE TIME.
