FOR VIRTUAL INTAKE APPOINTMENTS:
You will receive the intake paperwork to your email address. Once the intake paperwork is completed, your virtual intake appointment will be confirmed. If you do not receive the intake paperwork, please text 203-212-8842. We look forward to providing you with your treatment needs.
FOR PARENT EDUCATION PROGRAM CLASSES:
We will email you the registration documents. Once the registration forms are received, your registration will be complete and your place in the class will be confirmed. There are limited placements in the classes, therefore to secure a placement in the class, we recommend submitting the registration forms as soon as possible. If you do not receive the registration forms within 24 hours, please text 203-212-8842. PLEASE DO NOT REGISTER FOR A CLASS MORE THAN ONE TIME. If you need to change the date, or are unsure if you are registered, please text 203-212-8842. We look forward to having you attend our class.
